Frequently asked questions

What exactly does OSHA require on a lockout/tagout training record?

1910.147(c)(7)(iv): the employer certifies that employee training has been accomplished and is being kept up to date, and the certification contains each employee’s name and dates of training. That is the minimum. Adding the role, the trainer and the procedures covered is what makes the record useful when an inspector asks who is authorized on what.

Authorized vs. affected: does it change the training?

Yes. Authorized employees (who apply locks) must be trained to recognize hazardous energy sources, the type and magnitude of energy, and the methods for isolation and control. Affected employees need to understand the purpose and use of the procedure. Other employees need to know the procedure exists and not to restart locked-out equipment (1910.147(c)(7)(i)).

How often is LOTO retraining required?

OSHA sets no fixed interval for retraining itself; it is triggered by a job change, a new hazard, a procedure change, or inadequacies found. Separately, the employer must inspect each energy-control procedure at least annually and review it with the authorized employees (1910.147(c)(6)), which is why most sites refresh LOTO training yearly. Shield issues LOTO certificates for 3 years.

Does an online course satisfy the training?

The knowledge portion, yes. The employee also has to know your site’s specific procedures and equipment, which is why this record has a line for the procedures covered. Shield’s LOTO course covers the 1910.147 requirements; you add the site-specific walkthrough and log it here.
