Forklift Certification Record Tool

OSHA 29 CFR 1910.178(l)(6) says the employer must certify that each operator has been trained and evaluated, and the certification must include the operator’s name, the training date, the evaluation date, and the identity of the person who did the training or evaluation. Fill in those fields below and print, save as a PDF, or email yourself a copy for the file. No account needed.

What the record contains

Frequently asked questions

Is this an official OSHA form?

No. OSHA does not publish or require a specific form. 1910.178(l)(6) lists what the certification must contain; any format that holds those four items is acceptable. This tool puts them on one page in a layout inspectors are used to seeing.

Does a wallet card or online certificate replace this?

A course certificate proves the training portion; the employer still certifies the operator after the practical evaluation on their own equipment. Keep both. Shield certificates already record the evaluation date and evaluator once your supervisor logs the evaluation, so our customers can print this from the Employer Portal or the operator’s verification page instead.

How long do I keep it?

At least until the operator is re-evaluated (every 3 years at most) and, in practice, for as long as the operator works for you. Most employers keep the full training file.

Can the trainer and the evaluator be the same person?

Yes, as long as that person has the knowledge, training and experience to train operators and evaluate their competence (1910.178(l)(2)(iii)). When the training was an online course, the trainer line names the course provider and the evaluator is the person at your site who watched the operator drive.
